PIXMA MG3620 - Index Card Not Printing

My file prints fine on letter size. However, when I try to print it on a portrait 4x6 index card, scaled to fit, the printer grabs the card, the cartridges are moving, but the end result is a blank card.

PIXMA TR4722: Back door broke off while printing on card stock
