I'm considering purchasing an pre-owned ef400 2.8ii from a dealer in Japan. I've been reading that if something happens to the lens, I can't get it repaired from an authorized Canon repair center in the US. Is that correct?

Maybe! Call Canon and ask 1 (800) 652-2666.
It is a matter of them not wanting to repair it as they can't repair it if they don't have the specific parts.

Oh, BTW, if you want the reason for this Canon will always return it to its original factory specifications because Canon service warranties all work done. If Canon USA can not do that they won't repair it at all.

Before you lay out a significant amount of money confirm with Canon that they will repair grey market out of warranty products. There are myriad discussions on the web where this is reported, but best to confirm when it's your wallet involved.
I checked, and it looks like the Mk II version is still on the repair list.
