What would be the best set up for my 50D with 1.4 Extender II using Manual Mode for best results. Using with a100-400 USM Lens

When you say, "Set up", what did you have in mind?
The settings you choose are going to vary, based on the time of day, availability of light, whether the subject or subjects are moving or not etc.
Tell us a little bit about what you are trying to photograph.

Well first off using a tel-con doesn't mean you need M mode. Is there another reason you want M mode. Next question is which 100-400mm zoom do you have. The first model Canon 100-400m doesn't like a tel-con very much and I do not recommend it.
A better option is to crop in post editing. You can get a very good photo editor from Canon for free. DPP4 is a d/l form the Canon web site.

Use the amazing modes as is. It will be compensated through the lens. Manual is NOT the holy grail, though it comes in handy for tricky lighting. Tv is probably the mode you want so you can set shutter speed as a priority.
