I have run into this problem and not found a solution. I'm shooting on an R5C, main video files are going onto a CF Express card, proxy files are going to an SD Card. It records both formats as it should but the proxies have a different naming convention. For example:
main video file : A006C183_240329YW_CANON.MXF
Proxy file : MVIP0196.MP4

Have you reviewed pages 42 and 43 in the video manual. This assumes you are running FW v1.0.6.1
My understanding is this:
File naming is maintained when shooting RAW or XF-AVC > RAW or XF-AVC
.mp4 file naming is maintained .mp4 > .mp4
File naming is not maintained if RAW or XF-AVC > .mp4
You get the "P" designation indicating the clip is proxy, but naming convention is not maintained unless the above is followed.
