So while in stand by Low res, 4k 60fps, attached to an Atomos Ninja, What quality is actually being pushed through the HDMI?

The HDMI output resolution and frame rate are automatically adjusted to suit the movie recording quality to where the camera will output 4K at 60p if that is what you've set the camera to record.
