Maybe the problem is with the lens as you say. Because even with just the lens and the canon adapter (without the extender), it still doesn't work on the R body. However, it works well on the full frame bodys (not mirrorless)

The Canon Ts-e doesn't work in the Canon R and the Canon adapter. But still working on the Canon 6D (alone and with the extender 1.4)

I tried 2 Canon brand mount adapters, one simple, the other with a customizable ring, and one with a drop in filter Meike brand . I don't think that's the problem. Maybe its a lens issue like you say.
