Firmware shouldn't be an issue, but I would update it if the camera's not on the latest version just to be thorough. Is there anything connected to the camera's SDI or HDMI out ports? That's really the only thing I could think may be happening. If nothing is connected to the camera, I would suggest trying to reset the camera to default settings to see if that helps. The reset is in the System Setup (wrench) menu. If the issue still persists, then the camera would need to be sent into our factory service center.
