When I look at where my focus points are in DPP some show on the subject but that subject is slightly oof. I have been having inconsistent focus issues since I got this camera. Sometimes it's perfect so I'm not sure what I am doing wrong. I typically use EF 70-200mm f/2.8L IS III USM (with the canon adapter) but also just acquired the RF100-500mm F4.5-7.1 L IS USM.
I shoot mostly sports(softball, moving horses) using servo af, af one point,subject detect people, case 2 or 4 with tracking sensitivity set to negative, one shot af release priority set to focus.
Any input would greatly appreciated.

How were you holding the camera when you captured this photo? Were you using the rear screen or the viewfinder? It’s usually best to use the viewfinder as much as possible.

There seems to be some confusion with the settings described in the text and those shown in the screenshot from DPP info panel.
I'm not sure how much of a the full image that picture of the player is, but if you zoom in to 100% or more and they are still small in the frame you might not see all the detail you'd hope for. If the text on their shirt is sharp at that distance - you are using 472mm of the 100-500mm lens - I would expect the face to be too. The face looks a little less detailed since it's in shadow due to the overhead / slightly backlit position of the sun.
Do you also have initial servo AF pt for face + tracking set to something other than auto?
With AF case2 and AF case 4 with the tracking sensitivity set to -1 you slow the response of the camera to objects at different distances on the AF point. I would try changing to case 1, or leaving case 4 with its default tracking sensitivity set to 0.
