Trying to register a replacement printer and buy CarePak

Canon generously provided a new printer to replace the one which stopped working. I have been trying to register it and purchase a CarePak. I entered the info from the packing slip and the new serial number. I uploaded the packing slip. It still doesn't show up as registered. I am asked for a CarePak activation code, which I don't have. 

How do I register and how do I purchase the CarePak for this printer?
