Generally, this happens when there is a lot of ambient light and slower shutter speed, the flash freezes the action at first, but the extended shutter allows an ambient exposure that causes a blur.
But, other than auto-magically reducing the size of the image for EF-S lenses, the quality of the image is not affected with the adapter, there is no glass, it is simply a spacer.
