PIXMA TS9521C - smudged lines and faded bands

Whenever I print a document I get faded bands and smudged/uneven lines. See photo. I have done all the available maintenance options on my printer as well as replaced all the ink cartridges. Please help.

The printer is the Canon Pixma TS9521C
