Most likely not, but, generally they have a very low shutter count. The vast majority of refurbished cameras are retail returns. My refurbished 7D Mk II had 324 actuations on the shutter.

They could have. They can come from almost anywhere. But they get a Canon warranty.

Just make sure that you get your "refurbished" gear directly from Canon. When Canon says "refurbished", we know exactly what that means. When another vendor says it, it means whatever they want it to mean. You shouldn't run into a serious problem with major on-line vendors, but I do not know what their warranty offers are.
I bought a 6D when they had the refurbished one on sale. I opened the box, and it smelled like new. Like Ernie said, the gear comes with a Canon warranty, which in most cases is for one year. I have bought a couple of lenses from the Canon refurbished outlet. The lenses come as good as new, too.
