100-400 is not AFing anymore.
All started abruptly few weeks back.
It was an intermittent problem at first. Sometimes back to focusing once I remove the lens from body and reattach or play on / off with AF switch.
Camera doesn't seem to be the issue as it act the same with few other camera bodies too.
Before this I was able to manually focus at all rang but now I cannot focus anything farther than 5meter even manually. That is less than 5 meter, focusing ring rotates manually but gets stock at 5 meter and wont move any farther.
Any thoughts?

Sounds like a job for service to be honest. You appear to have a repeatable problem that moves with the lens.

Thanks.
I thought so.
Any guess as to how much it should cost me? if there is anything such as flat service rate?

Not sure for the costs, probably best for service to answer that one. Though to be honest I've mostly been pleased with the prices of service repairs.

Mine was not auto focusing about 7 or 8 years ago cost was $300+. That was was minimum repair cost then.
