The camera shows up in Windows file explorer, and works fine with Capture One Pro and other tethering apps.
I've tried uninstalling EOS Utility and tried several versions of the software.
I think the problem started after trying to use EOS Webcam Utility but am not sure.

The camera uses serial communication protocols. This means that it can only communicate with one app at a time. All other apps must be shut down, including any associated device drivers.

what device drivers might conflict? I've tried with no other apps open after a reboot.

Every app that talks to the camera, especially the EOS Webcam Utility. You need to disable it.
What OS are you using?
