Fax Machine F162002 L100 printing extra pages with patient information at the top page

Our pharmacy’s fax machine is receiving faxes correctly, but it consistently prints an extra page with the patient's full name at the top. We've reviewed the settings but haven’t been able to resolve the issue. Could you please assist us in troubleshooting this?

If these pages are printing with faxes, the machine is receiving, I suspect it has something to do with caller ID.
