EOS Utility images missing

I set up a tethered shoot (some artwork) as I have done hundreds of times.

I shot one image from the EOS Utility control dialog, it appeared in the destination directory.

In the past I've been able to shoot images from the camera, and they would go into the destination directory.  This time the subsequent images never appeared.. They are not on the card, and not in the configured destination directory.

It's super weird that the first one saved but the rest didn't. Usually, that implies the USB connection blinked for a second or the software lost focus. Definitely look at your destination folder settings again, sometimes a subfolder gets locked up.
