Hello,
I have ran through all the maintenance/cleaning troubleshooting and nothing has changed. I only use the black toner with the other toner low colors in the printer. The black toner is at 90% (less than a few months old and genuine) and I even took it out and shook it around to distribute the toner. I'm not sure what else to do here.
Any help is greatly appreciated.

The issue looks like a problem with the toner cartridge or there might be a problem with the fixing assembly on the printer. If you have already tried reloading and shaking the toner, you can try replacing the toner to see if it resolves the issue. If it does not, it looks like the printer needs service.
