Waiver form sample

Hello Everyone, 

 

I am looking to see if someone can help with providing a copy of a waiver form after taking pictures.  I live in the State of California to be more specific, in case laws change by the state. 

 

Any help in providing a template will be much appreciated.

Or are you refering to a Copyright Release form?  I can send you one of mine if it is.

If it is a model release you're looking for I highly recommend a smartphone app called Easy Release.  It's a standard model release but it's on your phone so it's always with you.  And you can snap a photo for ID right then and there.  It's accepted by Getty and most stock image companies.   Just practice with it before trying to use it the first time.

 

No point in it if this is a one time thing, but that app has more than paid for itself for me.

If it is a model release you're looking for I highly recommend a smartphone app called Easy Release.  It's a standard model release but it's on your phone so it's always with you.  And you can snap a photo for ID right then and there.  It's accepted by Getty and most stock image companies.   Just practice with it before trying to use it the first time.
