Unable to format SD card

Just purchased a new T5 Rebel 1200 D, also purchased a new SDXC, UHS-1 128 GB Sandisk ultra plus card. Camera states "can not access card" and yes the edit tab on the card is in edit position. Any suggestions?

Did you make the mistake of formatting the card in a computer that isn't SDXC compliant?

Did you format the card anywhere other than in the T5?

TTMartin, I thank you for your time. I descovered the MC chip was not able to seat completely into the adapter. Camera now functioning as advertised.
