You'll also want to fill out &lt;A href="https://downloads.canon.com/nw/cso/pdfs/repair-forms/fsc.pdf" target="_blank" rel="noopener"&gt;THIS&lt;/A&gt; form, print it out, along with your CarePAK confirmation email &amp;amp; Proof of Purchase (receipt) and you'll be on your way!&amp;nbsp;&lt;/P&gt;&lt;P&gt;Hope this helps!&lt;/P&gt;</description>
